This collaboration is set to transform the healthcare landscape for residents across Charter’s Midwest and East Coast senior housing communities, introducing comprehensive, in-residence medical services designed to significantly enhance quality of life and overall well-being.
So far, the partnership has brought concierge-style primary care services to 22 of Charter’s communities (with plans for expansion). Through the partnership, Curana Health’s physician-led care teams offer residents a wide array of services, including:
- Routine preventive primary care
- Urgent care services
- Palliative care
- 24/7 access to care teams
- Medication reconciliation
- Phlebotomy services
- Coordination of on-site imaging
- Comprehensive care coordination
Charter Senior Living is also participating in Curana Health’s Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO): a value-based care initiative designed to reward partners for improved health outcomes. By supporting care coordination activities and working to reduce unnecessary hospitalizations and emergency room visits, participants in Curana’s ACOs are eligible for shared savings. This approach not only promotes more efficient and effective healthcare delivery, but also reinforces Charter’s commitment to providing exceptional, resident-centered care.
